Summary of changes to Chronic Disease Management (CDM) Framework

Chronic disease management is a core role of general practice and GPs.

On 1 July 2025, a new framework for Medicare chronic disease management (CDM) items was introduced. Medicare items for chronic disease management are now simplified to a single item for plan preparation and second item for plan review. Transition arrangements will be in place to ensure continuity of care for patients currently under the existing CDM items.

The RACGP is committed to supporting GPs, practices, and patients through this transition.
