Chronic disease management is a core role of general practice and GPs. From 1 July 2025, there will be a new framework for Medicare chronic disease management (CDM) items. Medicare items for chronic disease management will be simplified to a single item for plan preparation and second item for plan review. The impact of the changes on annual funding for chronic disease management will depend on the billing approach for each patient.

Dr Michael Wright
RACGP President

MBBS MSC PhD FRACGP GAICD Dr Michael Wright is a general practitioner, health economist and health services researcher based in Sydney, Australia. He commenced as RACGP President on 21 November 2024. Michael is also an Associate Professor at the International Centre for Future Health Systems at the University of New South Wales. He combines clinical practice with strategic appointments and academic research analysing the effects of health policy on the quality and performance of primary care. Michael completed GP registrar training in Queensland and previously worked in London as a GP, where he completed his Masters in Public Health and was also a researcher at the London School of Hygiene and Tropical Medicine. Michael completed his PhD in Health Economics Research and Evaluation (CHERE) at the University of Technology Sydney (UTS) in 2019. His PhD investigated the impact of continuity of general practice care on health outcomes, and his research interest including growing the evidence base about the value of high-quality primary care, and health policy research into quality, efficiency and sustainability of heath services.

Dr Ramya Raman
GP, RACGP WA Chair and RACGP Vice-President

Ramya is a Perth based specialist general practitioner and a respected leader in primary care governance and advocacy. Ramya is a Director of The Garden Family Medical Clinic (Piara Waters) with a focus on women’s and children’s health, especially perinatal wellbeing. She received the 2020 Aspire Award to present her postnatal recovery research at WONCA Asia Pacific. As Discipline Head of General Practice at the University of Notre Dame’s School of Medicine, she promotes GP training and education. Ramya has also served as a clinical supervisor and examiner for RACGP. Ramya has published research in peer-reviewed journals and was RACGP WA’s 2020 GP of the Year.

Chris Smeed

Chris Smeed has seen first-hand the opportunities that can be realised through harnessing the power of practice data. Having worked previously as Business Manager at Inala Primary Care and as a surveyor for AGPAL, Chris provides expertise to the topic of business in General Practice.

Jessica White

Jessica has extensive experience across the primary healthcare sector, spending the last 15 years of her career focusing on eHealth, developing valued relationships with industry partners and is a strong customer advocate, using her experience and expertise to ensure that government and industry initiatives are delivered with customers always front of mind.
