Message from the Chair - Prof Charlotte Hespe


It's election time for the RACGP NSW&ACT Faculty Council again. The Faculty Council aims to represent all our regional GPs and advocate on the issues most important to you and your general practice community. If you don’t feel that the RACGP is currently representing your needs or concerns, then please put your hand up and consider nominating. Nominations for the upcoming vacant regional electorates close, Friday 18 August 2023. Further information regarding voting commencing will be available in due course. Please feel free to contact me if your region is not up for election this year but you’re still keen to have a say in matters relevant to your GP journey.
 
After months of intense lobbying and a disallowance motion on 60-day dispensing being abandoned in the senate, we’ll be able to write scripts for 60 days with five repeats for medications for chronic stable conditions from 1 September 2023. This is a victory for patients across Australia who will save valuable money on the medications they need at a time of high cost of living pressures.
 
Payroll tax in NSW and ACT continues to be at the forefront of all our minds and is an ongoing issue. With decisions emerging around the country in other states, such as recently securing payroll tax amnesty for practices across South Australia, your Executive Council members continue to seek further assurances for both NSW and ACT that general practices will not be subject to audits nor retrospective payments.
 
Last month we saw the introduction of MyMedicare, a new voluntary patient registration program that aims to strengthen the relationship between patients, their usual general practice, GP and primary care team. Keep an eye out for further information and updates on what this will look like going forward, as this is still a work-in-progress federally.
 
Due to the low vaccination rates for influenza across all ages in NSW this year, we’ve been having discussions with NSW Health about other ways to identify patients who are under-vaccinated but eligible for a free vaccination.  One possible idea is to send text messages to patients out of general practices along with a media campaign. Knowing how overworked GPs are at present, we’re keen to seek ideas and approaches that would not put a further strain on GP clinics, but at the same time, achieve higher vaccination rates. We’re keen to avoid any more deaths of young people due to poor vaccination rates.
 
On a happier note, we’ve completed fellowship ceremonies for new Fellows across NSW and ACT for 2023. I’d like to offer my congratulations to all new Fellows for choosing to train and be recognised as specialist GPs and invite everyone to connect with the New Fellows group. This enables you to meet up with other GPs at a similar stage of career progression and establish great social networking.
 
It’s hard to believe we’ve nearly reached the end of August and winter already. Remember to look after your health and well-being by doing activities that bring you joy and satisfaction!

60-day medicine dispensing: what does it mean?


From 1 September, GPs will be able to write scripts for 60 days with five repeats for medications for chronic stable conditions. The RACGP advocated strongly to secure this reform. It has significant benefits for patients and the health system and was first recommended by the Pharmaceutical Benefits Advisory Committee in 2018. It brings Australia into line with other countries including New Zealand, the US and Canada, where people already have access to multiple months of medication on a single prescription. It will save patients at least $180 a year and more if they’re taking multiple medicines, as well as reducing trips to get medicine, and freeing up GP appointments for other patients.

This reform will roll out in stages, with more medicines added until over 300 are on the list for 60-day scripts. From 1 September, the first stage of medicines eligible for 60-day scripts will support patients who are stable on their current treatment and living with ongoing health conditions including:

  • cardiovascular disease
  • Crohn disease
  • gout
  • heart failure
  • high cholesterol
  • hypertension
  • osteoporosis
  • ulcerative colitis.

The RACGP will provide detailed information for GPs and practice teams on 60-day medicine dispensing and fact sheets for patients to help prepare for the change on 1 September.

Child influenza vaccinations


Only one in five children under the age of five are vaccinated against influenza. Among school-aged children, it’s one in eight.
 
More children are presenting to the emergency department at the Sydney Children’s Hospital and The Children’s Hospital at Westmead.  This year, more than 500 children have presented at ED with confirmed influenza. Since May, over 16 children have been admitted to ICU.
 
The Sydney Children’s Hospitals Network is urging all clinicians to encourage child influenza vaccinations with parents and carers.

The Rural Procedural Grants Program is a government funded program that supports GPs with access to relevant educational activities to maintain or update their procedural and/or emergency medicine skills. 

You may be eligible for the Rural Procedural Grants Program if you provide procedural and/or emergency medicine services in unsupervised settings in rural and remote areas.  

Grants are up to $20,000 per financial year.

NSW Health podcast: STI and HIV care for transgender people in general practice


This podcast features sexual health specialist Dr Treeny Ooi, GPs Dr Lawrence Vaux and Dr Andrew Knight and consumer representative Eloise who will discuss how HIV and STI affect transgender people. It outlines the expectations of transgender individuals in relation to general practice, along with suggested practice modifications to better align with those expectations.

This engaging episode will also explore how to identify best practice language and sexual history taking for transgender patients. It also aims to identify training and resources that facilitate deeper understanding of providing safe and effective care to transgender clients in general practice and more importantly, hear about the patient experience.
 

Hyperemesis gravidarum – More than morning sickness


Hyperemesis gravidarum is a severe form of nausea and vomiting in pregnancy and is more than just morning sickness. It's emotionally, mentally and physically exhausting, and makes everyday life extremely hard.
 
MotherSafe is a free telephone service for women and health care professionals of NSW that can provide counselling and advice on medications to manage nausea and vomiting in pregnancy. Call 1800 647 848 or search ‘MotherSafe NSW’.
