If a candidate has encountered an exam incident that has had a significant effect upon their exam performance they can complete an incident report at the exam venue.
Alternatively, candidates can download the incident report form below and email the completed form to firstname.lastname@example.org within 2 business days of the exam. Only incident reports received within this timeframe will be considered.
All candidates who submit an incident report will receive an email acknowledging receipt of the report.
All incident reports are investigated and the impact on a candidate’s performance is considered by the Board of Censors (or delegate) prior to the release of results.
