Introduction to Healthy Habits

RACGP’s Healthy Habits is a free evidence-based digital tool that encourages patients to establish and sustain positive lifestyle changes.
 
Healthy Habits comprises a patient mobile app, website, clinician dashboard, resource hub and patient pathways conversation guides. The mobile app connects patients to their general practice dashboard so clinicians and primary care nurses can track their patients progress on achieving physical activity and nutrition goals and send messages of support.
 
This webinar will demonstrate the Healthy Habits tools and discuss some time-saving tips for implementing Healthy Habits in your practice. Nurse Practitioner Denise Lyons will also discuss how to identify suitable patients who would benefit from healthy habits and have a conversation with patients about making lifestyle changes using the 5 A’s framework.

Learning outcomes

  1. Describe the Healthy Habits tools and resources available to help patients to make positive lifestyle behaviour changes.
  2. Identify patients who are likely to benefit from using Healthy Habits.
  3. Implement strategies to engage with patients about making lifestyle change and using Healthy Habits.

Presenters

Emeritus Prof Mark Harris

Mark is an academic General Practitioner at the University of New South Wales, practising GP with the Asylum Seeker’s Centre here in Sydney and Chair of the Shaping a Healthy Australia Working Group at the RACGP who have advised on the development of the Healthy Habits tools.

Denise Lyons
Nurse Practitioner

Denise has been a nurse for more than 30 years, and began working in primary care 20 years ago. She currently works in General Practice as a Nurse Practitioner (NP), with people who range in age from infancy to aged, who have chronic and acute health problems. She has also been a long standing member of the Shaping a Healthy Australia Working Group at the RACGP who have advised on the development the Healthy Habits tools.
