Dr Jacquie Garton-Smith and Dr Mariam Bahemia discuss the use of the updated Advance Health Directive (AHD) in WA where there is currently low uptake of AHDs. Increased uptake of Advance Care Planning (ACP) and completion of AHDs is expected to reduce the disparity between treatment preferences and delivered care. This session explores the role of GPs in advance care planning, including supporting patients to prepare AHDs. Discover a new mini-audit activity for ACP and AHD that can be used to review whether opportunities to introduce ACP or AHD conversations into consultations, assessments and care plans are being maximized. Optional completion of the mini-audit will earn GPs 6 Measuring Outcomes CPD hours. This webinar is delivered in partnership with the WA Department of Health.

Dr Jacquie Garton-Smith has been the Royal Perth Hospital Liaison GP for 25 years. Jacquie is also the Clinical Lead for Primary Care Integration, Health Networks, Clinical Excellence Division in the Department of Health WA. In both these roles, Jacquie has advocated for good end of life care especially for people with chronic conditions, and has helped implement a range of successful initiatives in hospitals and WA Health. In 2019, she was on the WA Government’s Expert Ministerial Panel into Advance Health Directives.
