Managing key health issues in adult patients with an intellectual disability

Most full time GPs have at least 10 to 15 patients who are adults with intellectual disability who are considered to have the poorest health outcomes of any patient group in Australia. Managing key health issues in this group will make a difference to health outcomes and quality of life.
Learning outcomes
- Provide systems to identify/assess and then manage health issues with involvement of patients and person responsible.
- Incorporate and record identified health issues into a GP Chronic Condition Management plan to enable results driven healthcare.
- Apply the use of a template such as a Health Issues Related to Disability ( HIRD) to ensure key health issues are managed and reviewed.
- Involve the patient/person responsible and a wider multidisciplinary team using available Medicare supported initiatives such as GPCCMP/Case conferences/Medication Reviews and Mental Health plans.
- Evaluate the effective implementation of using a HIRD yearly and reviewed quarterly to maintain and improve a person with IDs quality of life

Dr John Crimmins is a GP and has been providing Inreach and phone consultation services for over 28 years and also provides NSW state-wide assessment services for adults with intellectual disabilities. John is also the current Chair of the Disability Specific Interest Group.
