This presentation will take GPs through future co-designed solutions for patients within our home care system. Silverchain provides care services for all ages and offers a key interface with the primary and acute health care sectors. Using novel models of care and ways of working along with case studies, the presentation will showcase Silverchain’s comprehensive in-home care across a range of clinical pathways and services.
Key points will include novel models of care, illustrated by case studies that show the clinical and socioeconomic value of home-based primary and community care, the advantages of our shared long-term relationships with all our patients, the use of technology to support improved communication, care coordination and the vision for delivery of high performing interdisciplinary teams.
