Practice Experience Program – Specialist Stream Satisfactory Completion Framework Guidance Document

1. Introduction

This guidance document provides further information on the Satisfactory Completion Framework as detailed in the Practice Experience Program – Specialist Stream Partially Comparable Exam Eligibility Policy, Practice Experience Program – Specialist Stream Partially Comparable Requirements for Fellowship Policy, and the Practice Experience Program – Specialist Stream Substantially Comparable Requirements for Fellowship Policy

2. Context

2.1 Practice Experience Program Specialist Stream (PEP SP) participants are required to complete the educational components of the program to a satisfactory standard as part of the requirements of Fellowship of the RACGP (FRACGP). The Practice Experience Program – Specialist Stream Satisfactory Completion Framework describes the criteria against which this requirement is measured.

2.2 Please note, in addition to the criteria for satisfactory completion (Clause 3), all of the below requirements need to be fulfilled:

  1. Participants nominated supervisor/s will complete a supervisor report that the participant will submit report to the PEP portal;
  2. Medical Educators (MEs) from the participant’s Training Organisation will complete the global assessment form after finalisation of the Work Based Assessment (WBA)components ; and
  3. RACGP MEs will complete the concordance form once the global assessment has been submitted.

2.3 If the RACGP deems the outcome of the concordance form as satisfactory, this means:

  1. Partially Comparable PEP SP participants can apply to sit the Fellowship Exams, only if they have met all other requirements as detailed in the Practice Experience Program - Partially Comparable Exam Eligibility Policy.
  2. Substantially Comparable PEP SP participants will be invited by the RACGP to apply for FRACGP if they have met all other requirements as detailed in the Practice Experience Program – Substantially Comparable Requirements for Fellowship Policy

3. Criteria for satisfactory completion

Workplace-based assessment (WBA)

Does not participate in any clinical assessments.

Completes less than four mini-CEX assessments during the six month term.

Completes mini-CEX assessments that the Training Organisation ME deems to be to an unsatisfactory standard.

Satisfactorily completes at least four mini-CEX assessments during the six month term.

 

 Does not complete at least three clinical case analyses during the six month term.

Satisfactorily completes at least three clinical case analyses during the six month term. These can be either random case analyses, case-based discussions, or a combination of both.

 

Does not complete a Multisource Feedback (MSF) process.

Completes a MSF process including self reflective exercise.

4. Glossary

Clinical case analysis

A hybrid assessment comprising oral questioning and reviewing clinical notes or case reports.

The PEP Specialist Stream offers two options for clinical case analysis:

  1. Case-based discussion (CBD) requires that participants submit three-four cases on the RACGP case submission template at least two weeks prior to the assessment. Participants can use the case reports that they have written as part of their learning unit completion. The assessor will review their cases and select two cases for discussion. If the cases are not at a satisfactory standard, then the assessor can return these to participant for revision.
  2. Random case analysis (RCA) is undertaken by the assessor randomly selecting patients seen by the participant in the preceding week. The assessor will need access to the participants appointment book and patient notes.

Mini-CEX assessment

Mini-Clinical Evaluation Exercise Assessment is a specific WBA tool used to assess those criteria best evaluated by direct observation of a participants performance in practice and provide feedback on their performance.
The Mini-CEX is flexible, allowing each assessment to be focused on one or more specific areas of the clinical encounter.  

Direct observation of consultations can occur via a variety of modalities:

  1. Direct observation of consultations in your practice – either face-to-face by an ME, using alternative modalities such as Zoom or Skype, or via review of recorded consultations.
  2. Direct observation of consultations with a simulated patient – either face-to-face by an ME or using alternative modalities such as Zoom or Skype

Multisource feedback

A well-recognised, valid and reliable method of assessing interpersonal and professional behaviour, development and clinical skills.

MSF has three components:

  1. a colleague feedback assessment tool and a self-assessment tool, known collectively as the Colleague Feedback Evaluation Tool (CFET)
  2. a patient assessment tool, the Doctors’ Interpersonal Skills Questionnaire (DISQ)
  3. a self-reflection component where you rate yourself against the questions in the CFET.

Workplace-based assessment (WBA)

The assessment of day-to-day working practices undertaken in the working environment. The WBA enables assessment of competencies in a real-world setting.
