This webinar supports practice teams to improve effectiveness in conducting health assessments with Aboriginal and Torres Strait Islander patients.
Since the Aboriginal and Torres Strait Islander health assessment item numbers were established, there has been a steady increase in their uptake. For these to be at their most effective, appropriate follow-up services and actions need to be implemented systematically.
The current evidence indicates that follow-up of Indigenous-specific health assessments is inconsistent and that improved health outcomes can be achieved when continuous and coordinated follow-up care is provided.
