From July 1, 2025, Australia is implementing changes to the Medicare chronic disease management (CDM) framework.

The key change is the replacement of the existing GP Management Plans (GPMP) and Team Care Arrangements (TCA) with a single, streamlined GP Chronic Condition Management Plan (GPCCMP).

This new framework aims to simplify processes and improve patient care by encouraging greater integration with MyMedicare and clarifying referral processes for allied health services.

Conduct an audit of your current approach to reviewing patients with an active Chronic Disease Management (CDM) plan (2hrs RP, 5hrs MO)

Under the new framework, regular structured reviews are essential both to achieve improved clinical outcomes and for MBS compliance. This activity will help you assess how consistently reviews are being conducted proactively and make improvements where needed to establish consistent review patterns that are aligned with the new framework.  

Record each step below as it’s completed in the CPD type listed next to the step or record this whole quality improvement activity once complete for Measuring outcomes CPD.

Step-by-step:

  1. Identify patients with active CDM plans – RP CPD  
  2. Record the number of reviews conducted in the past 12 months – RP CPD
  3. Set a baseline (eg % of plans with at least one review) – RP CPD
  4. Set a goal (e.g., increase reviews to every 3 months for 50% of patients) – RP CPD
  5. Track progress monthly using PMS reports or a Cubiko dashboard – MO CPD
  6. Analyze your data to determine whether any further changes are needed, and document your findings

This activity is expected to take around 3 months to complete and we recommend recording between 5 and 10 hours of MO CPD upon completion.

Here are some simple suggestions of activities you are likely already doing and the CPD activity type they each belong to.

Consider recording CPD when:

  • You familiarise yourself with the updated guidelines to educate your patients about how their care might be affected by the new arrangements – use Quick log to record this as education activities (EA) CPD.

  • You review the changes to the CDM framework and discuss the updates required in your practice as part of team meeting – use Quick log to log this individually or the GP-Led activity form found in the myCPD Log menu to record this as reviewing performance (RP) CPD.
