Capital Health Network, invites all General Practitioners from the ACT and surrounding areas to attend our GP Masterclass - Chronic Disease Prevention and Management.
The prevention and management of chronic diseases requires critical authentic approaches to the education of consumers and carers who use the principles of health literacy, teach-back, self-advocacy, self-management and decision support.
This approach extends across the patient journey into cross-sector chronic disease management programs, care planning processes and care coordination services.
The aim of such an education need should enhance patient activation and their involvement in their care.
