108874 - Quality Improvement in Primary Care Program

South Western Sydney Primary Health Network Limited

Available to GPs working within South Western Sydney. The Quality Improvement in Primary Care (QIPC) Program is a stepped approach to improving quality of patient care by using software, reports and supporting general practitioners, practice nurses and practice staff with education and resources to meet RACGP standards and best-practice goals for chronic disease.

Learning outcomes

  1. Identify practice systems to enhance patient care through better data management
  2. Identify patients indicated with chronic conditions who have not been coded in the clinical software
  3. Implement practice processes to increase recording of patients risk factors to align with RACGP guidelines
  4. Demonstrate better use of practice software to recall high risk patients
