With our current aging population, this health check ensures patients are being supported to live at home while maintaining the highest quality of life possible by ensuring adequate access to available health and community services.

Relevance to General Practice

It has been well established by evidence that Australia has an aging population and General Practice is increasingly faced with the responsibility to adequately treat, manage and prevent adverse outcomes in older people.
The Medicare-funded initiative of Health Assessments for patients over 75 years of age provides a vehicle for structured preventative care in this area, but the uptake of the initiative could be improved Nationally.
General Practitioners have identified a need for practical training and resources to assist with implementation of this initiative in practice. This course aims to address this need.
