The National Strategic Framework for chronic conditions supersedes the National Chronic Disease Strategy 2005 and associated National Service Improvement Frameworks.
The RACGP thanks the Department of Health for the opportunity to provide comment on the public consultation of the National Strategic Framework for Chronic Conditions (the Framework).
The Framework moves away from a disease specific approach, focussing on principles common across chronic disease management and prevention. This has particular relevance to general practice, as people with chronic conditions make up a significant proportion of the work carried out in general practice.
