Clinical audit quality improvement activity

This audit aims to identify, with the use of existing medical record software, Aboriginal and Torres Strait Islander adults with Chronic Kidney Disease. The audit process will allow GPs then to select five of these patients and to develop and implement strategies to assist these, and other patients, to improve their renal health and slow or prevent the progression to end stage kidney disease, cardiovascular disease or stroke.

On completion of this audit, participants will be able to:

  • identify the Aboriginal and Torres Strait Islander adult patients in the practice’s patient population who have  Chronic Kidney Disease.
  • select and evaluate five patients from this target group
  • develop strategies to assist these patients to delay progression of CKD and address risk factors for end-stage renal disease and cardiovascular disease
  • implement these strategies into their practice
  • evaluate the impact of the new processes on disease progression and monitoring of the selected patients
  • consider ways to improve the quality of care delivered to all Aboriginal and Torres Strait Islander patients with Chronic Kidney Disease

This quality improvement activity is eligible for 40 Category 1 QI&CPD points in the RACGP QI&CPD program and has been developed by the RACGP National Faculty of Aboriginal and Torres Strait Islander Health.
