Patient safety initiatives
Safety every time - Our general practice checklist
Have you ever given a patient the wrong vaccine?
Have you ever mistaken one medication's packaging for another?
Have you ever syringed the wrong ear?
Have you ever removed a lesion from the wrong site?
The Safety every time checklist has been developed by the RACGP to assist GPs and their practice teams minimise these sorts of risks and prevent adverse outcomes in general practice. The checklist is based on the Correct Patient, Correct Site, Correct Procedure protocols developed by the Australian Council for Safety and Quality in Health Care (ACSQHC) and the Royal Australasian College of Surgeons (RACS).
The Safety every time checklist is designed to be:
- reflective of procedures performed in the general practice setting
- easy to use, and
- helpful in minimising the risk of errors.
Enquiries about the checklist should be directed to the GP Advocacy and Support department at standards@racgp.org.au telephone 03 8699 0414, or fax 03 8699 0400.
