Certain of the following forms are intended solely for use by GPs applying for their own CPD. If you are a GP developing an activity for other GPs, you may be considered as a 'Education Provider'. In this case, please refer to forms under "Provider Forms and Guides". The forms intended for individual GPs only are the "Individual Application Form", "ALM Individual GP Application Form", "Higher Education Individual Application Form", and "Writing Peer Reviewed Article Application Form"
