Chronic condition self management guideline for GPs
The current initiatives relating to the management of chronic disease (e.g. new MBS items, co-ordinated care trials and curriculum changes to General Practice training) together with recent reforms to general practice (eg. Divisions of General Practice, vocational education) provide an ideal environment for the development and implementation of chronic disease self-management programs. The WA Research Unit of the RACGP has developed the Sharing Health Care (SHC) Guidelines with funding from the Department of Health and Aged Care. A wide consultation process has been used to gain feedback from stakeholders in varied settings.
- Part A - A4 desktop guide which incorporates a flow diagram and example careplan.
- Part B - An A5 guideline with intermediate amount of detail.
- Part C - An A4 manual with detail related to the theory of self management and several case examples.
