HealthSmart Family History Medical Chart
HealthSmart, a new magazine published by Reader's Digest, recently developed a family medical history chart for patient use. It is a simple chart that allows you to write down your family's medical history to take to your doctor. This can help your doctor identify if you are at increased risk of certain conditions. It divides conditions into cardiovascular, diabetes, cancer and other medical conditions. The package was prepared with assistance from the RACGP, Centre for Genetics Education, Menzies Research Institute and other local medical authorities.
